This page provides a checklist of each item you must complete in order to finalize your divorce.
- Pay the Oak Haven paperwork flat fee of $750:
Pay Flat Fee - Both parties sign the Stipulated Petition for Divorce (emailed to you)
During the 30-day waiting period, complete the following:
Non-Public Information Forms
Required Classes
Email your certificates of completion to Oak Haven Mediation
Income Verification
Email the following documents to Oak Haven Mediatiion
- Each party's two most recent pay stubs
- Each party's most recent W2
- The parties' most recent tax return, first two pages only
- If applicable, profit and loss statements from the parties' businesses
Once the 30-day waiting period has expired AND the parties have completed all of the above steps, you will be emailed the following documents to review and approve:
- Proposed Decree of Divorce
- Proposed Findings of Fact and Conclusions of Law
- Declaration of Income Verification
- Declaration of Jurisdiction and Grounds
- Request to Submit for Entry of Decree of Divorce
Once the "Final Documents" have been submitted, the Utah Courts have up to six weeks to review and approve the documents.
When the Courts approve the documents, you will be officially divorced. Oak Haven Mediation will email you a copy of your approved Decree of Divorce.
